Hi Year 7 Parents,

Our immunisation consent process has moved online - First Aid is no longer collecting paper forms.

Please complete the form by either:

• Clicking the link below, or

• Scanning the QR code in the attached letter and following the prompts.

The consent forms are sent directly to the Immunisation Nurses, ready for immunisation day. Additional information about the immunisations is also provided.

Please complete the form even if you do not consent to your child receiving the immunisation.

The School-based Immunisation Team will be visiting on 30 June for the Year 7 immunisations.

Throughout the 2025 school year, SBIP will provide the following immunisations;

Year 7 - 2 immunisations:
• Diphtheria, tetanus, pertussis (whooping cough) – (one injection) boosts immunity from a similar immunisation usually received during early childhood
• Human papillomavirus (HPV) – (one injection) protects against some strains of HPV and related types of cancer.

All students require parent/guardian consent before they can receive any vaccinations at school.
